There is no street address for this. The location pinned on the map is correct. The trout sculpture that is missing is the one that is located in the creek bed in along the Temescal Creek path between Clifton and Cavour. To locate the exact location in the creek bed there is a nearby cement pillar with a sighting tube that is pointed at the location the fish used to be at. If you continue along the path towards the DMV (towards Cavour) there is a cement table that has a copy of the fish so you can see what it looked like in its pre-stolen glory.
I believe that the bird statue (a Cooper's Hawk) was stolen from the table display once before and I think that the artist was able to replace it. If there is a previous service request related to that incident in the service request system it may have the artist's contact information.

The rainbow trout was located in the Temescal creekbed, and was immersed in the water. This part of the creek is behind the Davita property which has the address I reported. The creek has no street address. This is located somewhat parallel to Claremont Avenue, along the creek path closer to Clifton Street but there is no street address. It would be accessed from Clifton Street between Claremont Avenue and Miles Avenue.
